    Feeding South Florida's Afterschool Meals Interest Form

     

    Please complete this form if you are a community agency in Palm Beach, Broward, or Miami-Dade counties interested in becoming an Afterschool Meals site with Feeding South Florida.

    Agencies must have enough individuals available before, during and after meal times who will regularly provide meal counts/reporting information to Feeding South Florida.

  • Documentation of Licensing Status: Does your site currently possess one of the following that includes the site’s name and current address? 1. A Child Care License issued by the Department of Children & Families (DCF) or a local licensing agency, OR; 2. A Letter from DCF or Local Licensing Agency stating that licensure is not required, OR; 3. A Religious-Exempt Accreditation Certificate
  • Food Service Documentation: Does your Site possess a Food Sanitation Permit from the Florida Department of Health (FDOH) allowing for the operation of food service?
  • Documentation on Maximum Capacity: Does your Site maintain documentation identifying the maximum allowed occupancy of your space (usually obtained by a Fire Marshall or Certificate of Occupancy)?
  • Please note this is an interest form and submitting this information is not a guarantee or confirmation that FSF will provide Afterschool Meals to your Organization.
