Please complete this form to register for the Potential Partner Agency Information Session. The session will take place virtually. Attendance at this session is required before submitting an application.
PARTNERSHIP REQUIREMENTS
1. Your organization has a federally recognized 501(c)3 letter showing your tax exemption status as a nonprofit organization. (Proof of exemption is required).
o A copy of the 501c (3) determination letter or a letter of affiliation or affirmation must be sent to FSF before partnership is activated. Any changes in this designation must be reported to FSF at once and may affect partnership with FSF.
o An organization that is a church, or a program directly sponsored by a church, may use the non-profit designation of the parent affiliation of that church. In such cases, the organization must send a letter on the church’s letterhead affirming that the program is directly associated with that church and that the church is affiliated with the parent organization. In such cases where an Agency is being sponsored by an umbrella organization, the sponsoring Agency with the 501c (3) must sign the
Agency application in the designated place and sign off on the Partner Agency Guidebook.
2. Technological capability of running your organization (for example, computer and internet access/e-mail, ability to order online, fax machine, telephone, printer, scanner / copy machine, etc.). FSF is on a computer-based system and these elements are necessary for having a successful partnership with FSF.
3. The organization supplies food service to the ill, the needy, or infants/children.
4. Certification saying completion of a reputable food safety program, as approved by FSF.
5. Ability to sustain the food distribution program with funding and volunteers/staff.
6. Ability to distribute food at least once per week.
Please note the following:
o Meeting the minimum requirements does not guarantee approval of partnership. Other factors may be taken into consideration when approving agencies for partnership.
o Examples of other factors include but are not limited to hours and frequency of distribution; whether the organization location is in an underserved area; the number of FSF PAs already working in the area, and whether FSF partnership would duplicate services already provided.
o The completion of each step in the application process does not guarantee approval of partnership.
o To avoid improprieties of how product is used, FSF may not approve partnership applications with organizations that run a catering company or other for-profit business.
o To avoid duplication of resources and increase stewardship, FSF may not approve partnership applications with organizations that receive products from other nonprofit distribution organizations.
